This website is operated by Everglade Legacy Advisors (“we,” “us,” “our”). Insurance is offered through Simon Pullay, an independent insurance professional, National Producer Number 20128364.

We collect only what we need to run a qualification review and book a call. This site is not an insurance application and does not collect Social Security numbers or payment cards.

You provide

• Name, email address, and phone number
• ZIP code (we look up city and state)
• Product-specific answers: who coverage is for, age range, health questions (for example heart attack, stroke, or cancer in the last three years; tobacco; oxygen, dialysis, hospice, or nursing home), CDL and driving status, remaining mortgage range, and similar qualification items
• The appointment time you pick on Google Calendar
• Consent to be contacted by phone, email, or text

Collected automatically

• Pages viewed and funnel events (qualification started, step completed, lead submitted, calendar viewed, appointment marked booked)
• Approximate technical data such as IP address on our server fallback, and browser type
• A copy of your answers stored in your browser’s local storage so you can finish booking on that device — this is on your device, not a customer login

• Email the office a Review ID and your answers so we can prepare a call
• Let you schedule a 15-minute review on Google Calendar
• Contact you about the review you requested
• Measure which ads and pages lead to qualified appointments
• Protect the site (rate-limiting, abuse prevention) and comply with law

Health-related answers are used only to discuss possible issue types (for example simplified, graded, or guaranteed-issue) and to prepare the review. We do not use those answers to build advertising audiences.

We do not sell your personal information. We do not share it with nonaffiliates so they can market their own products to you.

We share information with service providers who help us run this site:

• FormSubmit — delivers the qualification form to office@evergladelegacyadvisors.com
• Google Calendar appointment scheduling — so you can pick a time. Google’s terms and privacy notice also apply to that booking
• Zippopotam.us — receives only the ZIP code to return city and state
• Website hosting and security providers
• Advertising and measurement tools, if installed (including Meta / Facebook and Instagram pixels). See Cookies and tracking

If you later apply for coverage, the issuing insurance company and its underwriting vendors receive application data under their privacy notices. This website does not send your questionnaire to a carrier automatically.

We may share information if required by law, to licensed partners when we cannot write a particular state ourselves, or with your direction.

When you submit a review request, you consent to receive calls, emails, or texts from Everglade Legacy Advisors at the number you provide, including by autodialer. Consent is not required to purchase. Message and data rates may apply.

To stop marketing texts, reply STOP. To opt out of marketing emails, reply to the message or write office@evergladelegacyadvisors.com. We may still send messages needed to complete a booking you already made.

This site uses essential cookies needed to deliver the pages. Funnel events are written to the browser’s dataLayer so we can tell a page view from a qualification start, a lead submit, and a booked appointment.

If a Meta Pixel (or similar ad pixel) is installed, those tools may collect device and interaction data and combine it with data they already have, to measure ads you saw on Facebook or Instagram. You can control ads through your Meta settings and browser controls. We do not pass your health answers into ad pixels as custom data fields.

As an insurance professional, we may collect nonpublic personal information (NPI) to provide a product or service you request. Categories include:

• Identifiers and contact data you submit
• Insurance and household information from the qualification questions
• Health-related answers you volunteer on the form

We share NPI with service providers who process the form and calendar, with insurance companies if you apply, and as required by law. We do not disclose NPI to nonaffiliates for them to market to you. You cannot opt out of sharing that is required to complete a review or application you requested.

Use office@evergladelegacyadvisors.com and include your name, phone, email, and Review ID if you have one (for example ELA-1234 or ELA-FE-1234).

Under the Colorado Privacy Act, Colorado consumers may have the right to access, correct, and delete personal data, and to opt out of targeted advertising, sale of personal data, and certain profiling. We do not sell personal data. If advertising pixels are active, you may opt out of targeted advertising by emailing us and by using your browser or platform controls.

To appeal a decision on a privacy request, reply to our email within a reasonable time and tell us why you disagree. If we deny the appeal, you may contact the Colorado Attorney General.

We do not sell personal information for money. If ad pixels are used to measure Facebook or Instagram campaigns, that may be “sharing” for cross-context behavioral advertising under California law. To opt out, email office@evergladelegacyadvisors.com with the subject line “Do Not Sell or Share.” Sensitive information such as health answers is used only to provide the insurance review you requested.

This site is for adults. We do not knowingly collect personal information from children under 18. If you believe a child submitted a form, email us and we will delete it.

Forms are sent over HTTPS. Lead emails go to our office inbox. No method of transmission or storage is completely secure.

We keep lead and appointment records as long as needed for the review you asked for, for licensing and complaints, and as required by law. You can clear local-storage copies in your own browser at any time.

We will post updates on this page and change the “Last updated” date. Material changes to how we use personal information will be reflected here before we rely on the new use where the law requires that.
